I'm not sure this would make a difference, but make sure the email template for the relevant message ("your accont has been activated") is published. If not published, the system may revert to the old method, and use the langauge files wording and text-only message (as was done before Mail templates were introduced in Joomla).

I found these extensions (Paid) but either might help track email being sent (at least confirm it is being sent, however the fact some emails are sent and received doesn't suggest to me a problem with the website or your method for the email set up).

I don't know that either would actually confirm receipt of a message, only that it has been sent from the web site/email server.

If you wanted more detail you'd probably need to access the mail server logs.

The other possibility is the final "your account is activated" is simply not been read by the user or has gone to the spam folder.
